It is the pneumatic actuator in an air brake: it turns air pressure into mechanical force that pushes a rod out and applies the brake. When the air is released the push rod should return fully. A rod that stays partly extended, or a chamber that applies unevenly compared with the one on the other end of the axle, is a reason to inspect it.
